Treadmill vs. Outdoor Running

Treadmill and overground running overlap, but systematic reviews show context-dependent biomechanical, physiological and performance differences.

Runner World interpretation: Treadmill and outdoor running are similar enough for many training purposes, but they are not perfectly interchangeable. Meta-analyses of crossover studies find that most biomechanical measures are comparable while some differ, and that physiological, perceptual and performance results change with speed, grade and test design. “As effective” therefore needs a defined outcome: maintaining a routine, matching oxygen cost and reproducing an outdoor performance test are different questions. Similarity does not mean identity

Peer-reviewed finding: A systematic review and meta-analysis found that many biomechanical measures were comparable between motorized treadmill and overground running, but it also identified differences in several sagittal-plane outcomes. Speed and grade change comparisons

Runner World interpretation: Claims that one fixed treadmill grade universally compensates for outdoor conditions go beyond the meta-analysis. Biomechanics vary by measure

Runner World interpretation: The biomechanics review compared spatiotemporal, kinematic, kinetic, muscle-activity and muscle-tendon outcomes. Some measures were similar and others differed, so no single finding can label all treadmill mechanics equivalent or inferior. Evidence basis: PMID 31802395

Performance tests are task specific

Practical or clinical guidance: For convenience or controlled pacing, exact equivalence may not be necessary. Practical or clinical guidance: The treadmill-versus-overground biomechanics review does not establish that one environment has lower injury risk.
